About Patient First Hospice and Palliative Care
Dementia Care Program supports people living with Alzheimer’s and other dementias through person-centered care. The team works to reduce agitation, ease behavioral symptoms, and maintain safety and dignity at each stage. Hospice care is provided wherever the patient resides, with 24/7 access to support for symptom changes.
