About Choice Hospice

Founded by a physical therapist after his grandfather’s experience with ALS, the provider centers care around serious neurological and chronic conditions. It offers in-home hospice, personal care, and therapy services, including specific support for people living with dementia or Alzheimer’s through its specialty programs. Nurses, aides, therapists, and chaplains work together to manage pain, assist with daily activities, and provide emotional and spiritual support. Bereavement services continue for up to thirteen months after a patient’s passing.
